  • Overview

    Provides primary case manager for assigned patients. Performs a variety of therapeutic and service assignments concerned with the treatment of alcoholism and other drug dependency in the primary, family and aftercare phases of the program.

     

    This is a full-time opportunity to work days/evenings in Minnetonka at the Hennepin County Home School. The schedule would be:

     

    Monday 8-4:30

    Tuesday 12-8:30

    Wednesday 8-4:30

    Thursday 10-6:30

    Friday 10:30-7

    Responsibilities

    • Communicates all referrals, external sources to facilitate patient care and progress in treatment
    • Meets with social workers, county staff, probation officers, employee assistance counselors or other concerned parties as needed, with appropriate consents
    • Provides day-to-day provision of primary counseling services in accordance with program policies and procedures
    • Contributes to the development, implementation and evaluation of the patients master treatment plan in collaboration with other team members
    • Documents patient information in EHR and/or other electronic system in accordance with program standards, department policies/procedures
    • Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures and standards
    • Actively participates in creating and implementing improvements
    • Performs other responsibilities as needed/assigned

    Qualifications

    • Bachelor's degree required. Must meet one of the Minnesota Department of Human Services qualifications categories for counselor
    • 30 hours of training in adolescent development required for Adolescent Program Staff
    • Licensed Alcohol and Drug Counselor (LADC) through the MN Board of Behavioral Health & Therapy required
    • Valid Basic Life Support (BLS) and First Aid Certification required
    • 1 to 2 years experience in chemical dependency treatment field required
    • Knowledge of application of self-help models (such as Alcoholics Anonymous)
    • Knowledge of the continuum of care for chemical dependency
    • Ability to provide guidance and feedback to help an employee or groups of employees strengthen their knowledge, skills and abilities to accomplish a task or solve a problem
    • Ability to use a computer and applications that are associated with performing basic work tasks (navigate in Windows, Outlook, etc)
    • Ability to provide counseling on an individual and group basis to achieve individual service plan goals. Ability to resolve issues. Ability to use interpersonal skills and methods to reduce or eliminate conflict
    • Ability to identify, assess, understand, and cope with a serious situation, especially from the moment it first occurs to the point that recovery procedures start
    • Provides patient educational information appropriate to the developmental level, learning needs, readiness, cultural values, and beliefs of patients and families
    • Identifies expected outcomes and develops individualized treatment plan of care
