As a Triage Care Connection nurse you will provide symptom management, general health and disease information to patients using computerized programs and the audio health library. Work with individuals to manage specific objectives for health care: self-management and improved well-being. Assist members of the community affected by cancer with support and referral to HealthEast Oncology services. Perform and provides linkage to care management activities. Act as a preceptor for new nursing and non-nursing staff.
Experience: 4 years recent health care experience, which includes at least 2 years clinical nursing experience. Previous teaching experience, mentor/preceptor skills.
Knowledge: Knowledge of utilization management, care management, and demand management.
