  • Overview

    At HealthEast, we are all called to improve the health of our neighbors and provide optimal health and well-being for our patients, our communities and ourselves.  As a Triage Care Connection nurse you will provide symptom management, general health and disease information to patients using computerized programs and the audio health library.  Work with individuals to manage specific objectives for health care:  self-management and improved well-being.  Assist members of the community affected by cancer with support and referral to HealthEast Oncology services.  Perform and provides linkage to care management activities.  Act as a preceptor for new nursing and non-nursing staff.

    Responsibilities

    • Maintains knowledge of, and complies with, all relevant laws, regulations and policies, procedures and standards.
    • Actively participates in creating and implementing improvements to achieve clinical, satisfaction and/or efficiency outcomes.
    • Answers incoming calls for HealthEast OneCall and performs nurse triage, general health, and disease information. Accurately assess callers needs and obtains and communicates appropriate information related to their symptom(s).
    • Utilizes the Audio Health Library and the General Health Information. Maintains comprehensive knowledge of triage protocols and Care System best practice guidelines and pathways, and utilizes those tools as appropriate.
    • Acts as a link to Care Management Services for all enrolled populations members and those requesting assistance through the Cancer Care Line.
    • Processes out-of-network requests according to policy and procedure. Maintains comprehensive knowledge of network providers and services, and general knowledge of clients benefit plans and claims administration.
    • Establishes and maintains communication with providers, care managers, and others to facilitate the appropriate management of care for callers to OneCall. Maintains partnership with HealthEast Oncology Services, enabling Triage Nurse to refer those affected by cancer to appropriate services and support groups.
    • Seeks input from providers, OneCall Physician Lead, HCI Director, and others as needed to assure that medical review issues are addressed.
    • Maintains documentation to support OneCall activity and generates reports as requested.

    Qualifications

    Education:
    Essential:

    • Associate's Degree
      Associate's degree in Nursing required. Bachelor's degree preferred.

    • Description: BLS

    Experience:  4 years recent health care experience, which includes at least 2 years clinical nursing experience. Previous teaching experience, mentor/preceptor skills.

    Knowledge:  Knowledge of utilization management, care management, and demand management.

    • Computer Skills Basic: Ability to use a computer and applications that are associated with performing basic work tasks (use a mouse, navigate in Windows, Outlook, etc).

    • Interpersonal Communication: Skilled in developing effective rapport with customers, co-workers, or families, actively listening to develop a positive connection.
    • Nursing Process: Utilizes the nursing process to provide holistic, compassionate, safe, high quality, population based care.
    • Patient Safety: Demonstrates practices that support safety with recognition of variations in populations served, preventing harm to patients, families, employees, and themselves.
    • Professional Practice: Demonstrates behavior consistent with the standards, scope of practice, ethics, and characteristics of a licensed professional.
