Be part of a team that is transforming primary care into what patients really want and need! HeathEast’s Clinic Care Coordination department is a Patient Centered Medical Home Program serving all of the HealthEast Clinics. This team consists of twenty five Care Guides who help each patient navigate their health care needs throughout the contiuum of care. As non-licensed professionals these Care Guides depend on our licensed teammates for items outside their scope. We have four Registered Nurses that support this team and work with our medically complex patients. You'll enjoy Monday through Friday day shift hours in an outpatient clinic setting. You will challenge your nursing skills and knowledge through comprehensive clinical chart reviews, meeting with the patient for an hour long assessment, and helping to chronic care manage patients who see mulitple specialists.
Experience: 3 to 5 years of experience working with health plans, clinic systems, or general health care environment with a focus of public health or care coordination preferred.
Knowledge: Knowledge of Medicare, Medical Assistance and Federal waiver programs necessary. ;Knowledge of community service organizations, agencies, programs and funding sources.
