  • Overview

    Supervise the total care management of clients, including case management as appropriate, utilizing the comprehensive base of physical therapy theory and process, inlcuding: assessment, evaluation, observation, planning, and implementation and performance of medically prescribed procedures and modalities, as evidenced through documentation. Provides advisory service to other professional workers who share the responsibility for the client's care.


    This is a full-time opportunity to work days, including rotating wekeends (approximately 1 out of every 6th Saturday and Sunday), and holidays (approximately 1 per year).


    Must have valid driver's license, auto insurance, and acceptable driving record.


    • Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures and standards
    • Actively participates in creating and implementing improvements
    • Performs client care and case management based on HealthEast Home Care's conceptual framework of practice to ensure effective and appropriate home care services
    • Directs, plans and initiates appropriate action autonomously and responsively in home care situations
    • Manages, supervises and instructs family, clients and/or other home health care professionals in appropriate physical therapy techniques and treatments, and appropriate client cares
    • Provides advisory and consultation services to staff to ensure quality care


    • Bachelor's degree in Physical Therapy required
    • Licensed Physical Therapist (PT) through MN Board of Physical Therapy
    • 1 to 2 years of Physical Therapy experience preferred
    • Must have reliable transportation to and from patient homes
    • Strong understanding of Physical Therapy principals and practices
    • Ability to life and transfer clients using appropriate equipment
    • Knowledge regarding Meidcare/Medicaid and third-party payors
    • Computer Skills Basic: Ability to use a computer and applications that are associated with performing basic work tasks (navigate in Windows, Outlook, etc)
    • Interpersonal Communication: Skilled in developing effective rapport with customers, co-workers, or families, actively listening to develop a positive connection
    • Patient Care Delivery: Observes and collects pertinent data and/or clinical findings to enhance patient outcomes
    • Patient Education: Provides patient educational information appropriate to the developmental level, learning needs, readiness, cultural values, and beliefs of patients and families
    • Patient Safety: Demonstrates practices that support safety with recognition of variations in populations served, preventing harm to patients, families, employees, and themselves
    • Professional Practice: Demonstrates behavior consistent with the standards, scope of practice, ethics, and characteristics of a licensed professional
    • Written Communication: Ability to organize and express information and ideas in written form to individuals as well as groups. Construct messages that are clear and convincing
